A patient transport provider operating across Norfolk and Waveney has been rated inadequate by the Care Quality Commission.
Inspectors found staff lacked specialist training and managers failed to respond properly to safety concerns.
Health Transportation Group, based in Norwich, disputes the findings and says it's launched a formal complaint.
